Virtual wards provide an alternative option for those who are frail, elderly, or with acute respiratory infections, who become unwell and would normally be spending time in hospital.
Our Virtual Frailty Ward Team, led by Team Leader, Emma Vant, see around 100 referrals each month into the service. They have been sharing information with wards about the support they can offer.
Emma said: "Virtual wards are run by a multi-disciplinary consultant-led team. They prevent both avoidable admission into hospital, as well as earlier discharge to the place our patients call home. They also help keep the beds in our hospitals free for our most acutely unwell patients.
"Other benefits include a reduced risk of delirium and deconditioning, as well as reduced healthcare acquired infections.
"Keeping frail people in their familiar surroundings with the necessary care and support from hospital and community services is likely to improve their outcome."
