From today, Monday, 15th September, our Risk Register is now hosted within InPhase - our incident reporting and management software.

The only thing that has changed is how you access the Risk application, which is through InPhase. The questions are all exactly the same.

An in-depth SOP on using the system is attached below.

Access to the old application (accessed via non clinical apps on the desktop) will be left as read only for two weeks. After this time, back ups of the data will be deleted from the server. If you need access to the read only or notice any of your risks are missing after Sunday, 14th September, please contact: InPhase.admin@cht.nhs.uk

Drop in information sessions this week

If you have questions, join our Risk Managers at an information session this week. You can copy and paste the date that suits into your Outlook Calendar.

All sessions are between 12:00 and 13:00.
