From next Monday, 15th September our Risk Register will be hosted within InPhase - our incident reporting and management software.

Access to the current application (accessed via non clinical apps on the desktop) will no longer be available from Sunday,14th September at midnight. The InPhase application will be live from 08:00 on Monday, 15th September 2025.

The current Risk Register application will be left as read only for two weeks. After this time, back ups of the data will be deleted from the server. If you need access to the read only or notice any of your risks are missing after 14th September, please contact: InPhase.admin@cht.nhs.uk

Will anything be different in the new Risk application?

The only thing that will change is that you will access the Risk application through InPhase. The questions are all exactly the same.
