Last week our Children's and Adult Diabetes Teams presented their work at the Regional Diabetes Seamless Transition Celebration event in Leeds.
The team shared how over the past year they have completed the West Yorkshire Seamless Diabetes Transition Training. The training is a year long programme which was fully funded accredited training for specialist paediatric and adult diabetes teams.
As part of the training the team have introduced a six month trial where Blosm attend to talk to our young people aged 16-18 who attend the transition clinic at both CRH and HRI. This clinic is also attended by members of both the Paediatric and Adult teams. The trial will be evaluated and the patient voice used to help further develop transition clinic services.
Clinical Nurse Manager, Sarah Schorah, said: "Well done to the teams who have completed the training and worked so well together over the past 12 months. Special thanks also goes to Paediatric Diabetes Specialist Nurse, Sasha Wright, who was the 'seamless champion' and was responsible for helping all team members in completing their training.
"It was great to be able to present what we have done at CHFT with this training. Other regional teams were extremely impressed with the involvement of having Blosm at transition clinics.
"I feel this training has brought different teams together, and encompasses the CHFT four pillars, whilst investing today for our adults of tomorrow."
