Here is a rundown of policies and guidance which apply to colleagues working in the Community Healthcare Division.
NEW - Urgent Community Response (UCR) Team for Medicines Administration Policy
This policy replaces previous Domiciliary Procedures. This is for all colleagues providing medication administration to patients at home.
New Community Delirium Pathway
A new Community Delirium Pathway is available on the CHFT Policies and Documents Library
The aim is to identify, assess and treat a person with delirium within their own home environment and avoid a hospital admission that may lead to distress and complications.
The pathway was developed in conjunction with Consultant Geriatrician, Dr VanBentum, Lead Matron for Dementia, Laura Doyle and Local Care Direct.
The care of people with delirium (acute confusion) is one of the common clinical conditions within NHS England’s (2022) Urgent Community Response vision. Within CHFT we have an Urgent Community Response team with Advanced Clinical Practitioners (ACPs) who will assess the person and a thriving virtual ward team to monitor their condition at home.
When Local Care Direct receive a referral (from a healthcare professional) they will ask a single delirium question as part of their triage process - 'Has the person being more confused or more drowsy than usual?'. If the person has no other red flags for an emergency condition that requires immediate hospital admission the case will be passed to the Urgent Community Response Team for an ACP to visit within two hours. The ACP will then assess the patient to confirm the presence of delirium and initiate and management plan.
Updated guidelines for the management of lower limb ulceration
Guidelines for the management of lower limb ulceration have been updated and can be found on the Policies and Documents Library by searching 'lower limb'.
The guidelines have been updated to reflect national policies and evidence-based practice and should be used as an information toolkit to guide the management of patients presenting with lower limb abnormalities.
