InPhase, our new Learn from Patient Safety Events (LFPSE) incident reporting and management system replaced Datix last month.

Our Quality and Safety Team have organised a series of lunchtime drop-in sessions on MS Teams. Colleagues who may be having issues logging in, or seeing their incidents or complaints are welcome to join. There will be a high level overview of the system, how to search and filter or find specific incidents. There will also be a quick look through the dashboards.

Colleagues can also email the team with issues. Just drop them a line and they'll be in touch to support, or call you to chat through. Email them on inphase.admin@cht.nhs.uk
