A huge well done to Wards 5 and 6 at HRI who have recently smashed the 90% target for delirium and dementia screening on EPR. 

Prevention Of Delirium Co-ordinator, Rachel Bennett, was so impressed that she paid both wards a visit to give them each a box of Heroes chocolates and a thank you card from Dementia and Delirium Matron, Laura Doyle.

Ward 5 Sister, Amy Toomes, has been a driving force on their ward for timely screening. She said: "The ward sisters have been working alongside our amazing Engagement Support Workers, Lucy Dickinson and Nicola Wrigley, to ensure timely delirium and dementia screenings are completed using live data from KP+. 

"Knowing the screens can prompt investigations or follow-up appointments has been the key to us being determined that all patients that require them are completed. 

"Both patients and colleagues have benefited from our improved compliance, as the assessment often turns into a chit-chat about anything and everything! Working together has definitely got results!"
